Health is about more than a clinic visit. Avenue 360's supportive services address the social drivers of health — connecting patients to housing, benefits navigation, transportation, translation, and Ryan White HIV services. In 2023 alone, we helped coordinate stable housing for more than 520 people.
Who it's for: Patients who need help navigating housing, benefits, or social services — no one is turned away for inability to pay.

Community Health Workers
Cultural liaisons who provide outreach, patient navigation, follow-up, and community health education.
Health Advocacy
Connecting people experiencing homelessness to services, plus free screenings for blood pressure, glucose, HIV, Hepatitis C, and Syphilis.
Housing Support
Permanent Supportive Housing with rent subsidies and case management, and SOAR help applying for SSDI/SSI benefits.

Comprehensive HIV care including testing, treatment, PrEP/PEP access, and emergency financial assistance.
